Our Community

Palisade Healthcare Community is located in scenic Garretson, SD, a town rich in railroad history, Wild West lore, and natural beauty. Whether you need transitional care following a hospitalization or require long-term care because it’s no longer safe to stay at home, we offer a peaceful retreat and place of healing where you’ll receive high-quality care with compassion and kindness.

Our Approach

Working closely with your physician, family and other healthcare providers, our team creates your individualized care program. Once you’re here, our medical director, primary physician and their teams visit frequently, assessing your current health status and adjusting your care plan as needed.

We are one of the few residences in the area that offers peritoneal dialysis. This allows you to complete your treatments in the comfort of our homey setting instead of trekking to distant Sioux Falls several times a week. For those needing short-term care following hospitalization or illness at home, our physical, occupational and speech therapy teams are ready to accelerate your recovery in our fully equipped gym. We’ll get you home fast! Our discharge-to-home rate is 85%.
