Working closely with your physician, family and other healthcare providers, our team creates your individualized care program. Once you’re here, our medical director, primary physician and their teams visit frequently, assessing your current health status and adjusting your care plan as needed.

Fountain Springs’ dedicated post-acute unit includes a spacious, fully equipped gym is the cornerstone of our exceptional physical and occupational therapy program. Our therapists and staff take pride in watching you regain independence and cheer you on through every step of the transitional care process.
