Our Community

Set amidst the small-town charm of Grand Rapids within walking to fine dining and shopping, Evergreen Terrace provides high-quality and compassionate care. Whether you need transitional care following a hospitalization or require long-term care because it’s no longer safe to stay at home, we offer a peaceful retreat and place of healing.

Our Approach

Working closely with your physician, family and other healthcare providers, our team creates your individualized care program. Once you’re here, our medical director, primary physician and their teams visit frequently, assessing your current health status, and adjusting your care plan as needed.

Evergreen’s skilled nursing and rehabilitation facility has a dedicated post-acute unit with private rooms and a separate dining area for rehab residents. We excel in complex services, including ventilator and respiratory care. Because of our trusted relationship with the area’s acute-care providers and healthcare networks, we’re able to accept most patients with complex medical needs at any time of the day or week. After I broke my hip, I came to Evergreen to rehabilitate. The therapists, Jim, Kelsey and Sue, were excellent. They helped me get up on feet, and—little by little—got me walking again. By the time I left, I was a lot more independent, and that’s been a big plus at the assisted living community where I now live.

They rest of the staff was great, too. They all treated me so well. If I needed something, they got it for me pretty quickly. My room had a window by the birdfeeder, so I enjoyed watching the birds and the deer, and seeing the sun shining. The meals and desserts were delicious. Beef stroganoff was my favorite. Good coffee, too.

Overall, my rehab experience was much better than I thought it would be. Everyone was so nice to me, and I enjoyed getting to know the other residents. I highly recommend Evergreen. It’s a great place to recover.

I had an injury that needed a wound vac, and Evergreen was the only rehab equipped to take care of me. My therapists were a real hoot. I really enjoyed my sessions with them. Sometimes, after my therapy, I’d just stop in to visit with them. They got me up and walking. And although I still use my wheelchair, I’m making good progress. Now that I’m home again, I’m doing great sitting outside and getting some gardening done.

The nurses and aides gave me the best care, and I made friends I hope to keep. I had fun talking with the staff and other residents, and will miss everyone but I’ll come visit.
