Our Community

Whether you need transitional care following a hospitalization or require long-term care because it’s no longer safe to stay at home, Beartooth offers a peaceful retreat and place of healing. Located on the edge of Columbus, MT, with spectacular views of the Yellowstone River and snow-capped Beartooth Mountains, our award-winning staff serves you with dignity, kindness and respect.

Our Approach

Working closely with your physician, family and other healthcare providers, our team creates your individualized care program. Once you’re here, our medical director, primary physician and their teams visit frequently, assessing your current health status, and adjusting your care plan as needed.

Beartooth’s skilled nursing and rehabilitation facility has a dedicated post-acute unit and five Medicare rooms. We also offer a dementia care wing that includes a separate secure unit. Our medical capabilities and services include:
